If your annual Membership renewal has recently passed (within the last 7 days), but you now longer wish to be a Member, please contact the Mayfield administration team via this form and we will look to refund your Membership. The funds should be returned to you within the next 5 days.
Please note that the Mayfield Clinic Membership is set to automatically renew each year unless we are otherwise advised. This is documented in our Membership terms and conditions accepted upon sign up.
We will of course be delighted to help you in the future should you have any need of our services; you are welcome to use our services as a non-member.
